Veterinary Consent Form

In accordance with the Veterinary Surgeons Act 1966, and the Exemption Orders Act 2015, before I can treat your dog, I will need you to have your vet sign my Veterinary Consent form. This can then be emailed to me in advance or available at your first appointment.

I will not work on a dog that has not been cleared by a vet to receive therapeutic canine massage.
